Forms to be filled out by you and turned in:

  • School permission slip to dispense medication to students (NEW)
  • AstroCamp Student Application-Medical Form *** Please note that all medications, even over the counter medication must be accompanied with DOCTOR'S ORDERS and must be turned into the Health Office prior to the trip.  Additionally, no medications will be taken without an adult dispensing the medication to the student at the camp.  Students CANNOT have any medications in their luggage.

Important Preparation Dates - 2013

  • Fundraising Opportunity Starts:  Monday, January 7
  • Fundraiser Orders and Money Due to Ms. Bassine in the Main Office:  Friday, January 18
  • Final Payment Due: Monday, January 14
  • Parent/Student Meeting (turn in paperwork) in the Adams Gym:  6pm on Tuesday, January 15
  • AstroCamp Weekend: Friday - Sunday, February 8-10

What is AstroCamp?

AstroCamp is a science camp in Idyllwild that will provide our students, your children, with an amazing experience that has the potential to not only increase their knowledge and interest in science, but may spark a life-long passion for learning.

Who is eligible to participate in AstroCamp?

All Eighth Graders are eligible to participate in AstroCamp.  Space is limited, so reservation are accepted on a first-come basis.  A parent letter is sent home usually in October with a tear-off section indicating interest.  Filling out this form and returning it to the Adams Office with a deposit holds a spot for your child.

How much does AstroCamp Cost?

AstroCamp is around $200 per student.  The cost varies by year, so please refer to the Parent Letter for the exact cost.  There will be a fundraising opportunity for those who either need financial assistance or prefer their students to "earn" their way to camp.

Fundraiser Information

MIXED BAG DESIGNS

Eco-friendly MIXED BAG DESIGNS (www.mixedbagdesigns.com) fundraiser is to help raise funds for 8th graders to attend AstroCamp. This is an easy & profitable way to help cover the cost. Increase your profits by obtaining orders from the catalog & online orders from friends, relatives and co-workers. If you get people to order online you will not have to collect money or deliver the products but they will pay shipping. Click on “online store” at www.mixedbagdesigns.com to place your order, it’s that EASY!!!!! Make sure they indicate your student's name when they order online so your student will receive credit.

EACH STUDENT’S ACCOUNT WILL BE CREDITED ACCORDING TO WHAT THEY SELL.
STUDENT WILL RECEIVE - 40% (before sales tax) PROFIT FROM ALL RETAIL SALES TOWARDS THEIR TRIP
Example: Sell $100 = $40 credit towards trip / Sell $200 = $80 towards trip, etc.

Instructions:

1) The seller’s name should be on all order forms and on order envelope.

2) Please add 9% sales tax.

3) Send your order form (we will place catalog orders for you, you do not need to) & payment to the school office January 18, 2013. Send cash and/or checks made out to ADAMS MIDDLE SCHOOL

4) Keep the bottom copy of your order form.

5) Items in the catalog marked with a black X thru them are not available to order.
