We understand that the middle school years can be a challenging time for students as they navigate a larger campus and discover who they are and where they fit in. Our students have always been great at identifying unsafe situations or sharing with us when they are mistreated; however, we understand that students may hesitate to come forward or share information in fear of retaliation. Our current practice allows our students to report concerns (like a friend in danger), incidents, and mistreatment by filling out an Incident Form available to any student in the front office.
In an effort to provide our students an additional means by which to report, Adams Middle School will begin implementing the Adams Incident Tip Sheet via a Google Form. This form can be filled out anonymously; however, we encourage our students to identify themselves so we can follow up with them when an incident occurs and is reported. When this Google Form is filled out, our administration team will be notified and will be able to review the report and take appropriate action.
As a tip reporting system, it is imperative that students clearly understand the importance of using it responsibly and appropriately. Students will be taught the appropriate use of this form through their Advisement class during the week of October 10th - 14th. Students will also be informed on the misuse of the reporting form and what the repercussions of misuse would be, including possible consequences. Families and parents can also use the form to report incidents, if needed. However, we encourage our students to be self-advocates.
Students will be taught the key points below:
- If this is a life-threatening emergency, please call 911 instead of using this form.
- Please know that while on campus during school hours you may come directly to the main office for immediate in-person support with an administrator or counselor.
- We use paper-based incident report forms in our office to report concerns that can be used in lieu of this online form.
- This digital form may be used to report a concern with or without identifying yourself. Please note that if you would like school staff to follow-up with you directly, you should provide your contact information. If you prefer to remain anonymous, that is okay too.
